Een correcte diagnose is essentieel om de patiënten te identificeren en ziekteprogressie te vermijden1

Bij ATTR‑amyloïdose bedraagt de tijd tot diagnose vaak meer dan 3 jaar. Onafhankelijk van het fenotype (CM of PN)2


17% van de patiënten met erfelijke ATTR-amyloïdose heeft meer dan 5 artsen geraadpleegd voor ze de juiste diagnose kregen3

Patiënten met ATTR-amyloïdose worden vaak miskend

Stappen in de diagnose van ATTR-amyloïdose

*Ecg: Discordantie tussen de gemeten QRS-spanning en de dikte van de wand van het linkerventrikel op beeldvorming, geleidingsstoornissen en ritmestoornissen9-11


Echocardiografie: Verdikking van de linkerventrikel, diastolische disfunctie van graad 2 of hoger, vermindering van de algemene longitudinale strain met desynchronisatie van de contractiliteit in de apicale segmenten (‘relative apical sparing’)9-11


hart-MRI (CMR): Verhoging van het extracellulaire volume en diffuse late gadolinium-aankleuring.9-11


§Bij hartamyloïdose lichte ketens (AL-CM) kan het beeld vergelijkbaar zijn met dat bij ATTR-CM, maar het wordt veroorzaakt door een plasmocytaire dyscrasie. Het moet worden uitgesloten met een screening op monoklonale eiwitten via sFLC, SIFE en UIFE.9-11


Als de cardiale scintigrafie negatief of twijfelachtig is en er een groot klinisch vermoeden is, moet een biopsie worden overwogen.9-11
